If you have completed your Communicable Disease Surveillance (CDS) requirements, we invite you to share your feedback about your experience with the CDS process, including the CDS website instructions, completing your medical requirements, and the support services available throughout the process.
The survey is conducted by NC’s Student Health & Counselling Services with technical support from NC’s Institutional Analytics & Reporting (IAR).

Purpose of the Survey
Your feedback will help Niagara College continue to enhance the Communicable Disease Surveillance (CDS) experience and ensure that the information, resources, and support available to students continue to meet their needs.
The survey period is October 05, 2026 – December 18, 2026.
Eligibility to Participate
Students who have completed their CDS medical requirements and received clearance confirmation are eligible and encouraged to participate in the survey.
Participation in the survey is voluntary and anonymous.
What Participation Involves
Participation involves completing an online and anonymous survey through NC Engage without logging into the app.
Choosing whether or not to participate will have no effect on your status at Niagara College.
The survey will take less than 5 minutes to complete.
The survey asks about your experience with:
- following CDS website instructions;
- completing your CDS medical requirements;
- participating in CDS support services such as open houses, in-class presentations, virtual presentations, and pop-up drop-in sessions;
- the helpfulness of the support you received; and
- the frequency of pop-up support sessions.
The survey also includes demographic questions to help Niagara College better understand the feedback received.
Survey Incentives
Students who complete the Communicable Disease Surveillance (CDS) Feedback Survey may enter a separate prize draw for a chance to win one of two $25 Amazon gift cards, subject to the eligibility requirements below.
The prize draw will take place on December 19, 2026. Students who wish to enter the prize draw will be directed to a separate Prize Draw Entry Form after submitting the CDS Feedback Survey.

Anonymity and Privacy
The CDS Feedback Survey is anonymous. The survey will not ask you to provide your name or NC student email address for the purpose of entering the prize draw.
If you choose to enter the prize draw after completing the survey, you will be directed to a separate prize-draw form where your NC student email address will be collected. Information provided through the prize-draw form cannot be linked to your CDS Feedback Survey responses.
Survey results will be analyzed and reported in aggregate wherever possible.
Information collected through the survey will be used for Niagara College purposes related to evaluating and improving the CDS process and student supports.

Alternative Delivery Options
Students requiring alternative delivery options to complete the survey may:
- Have the assistance of an aide or an assistant to complete the survey (i.e. reader, scribe, etc.);
- Schedule to have the survey read or scribed online through the test centre;
- Schedule a telephone survey.
