The process to submit your Communicable Disease Surveillance information will be different depending on what program you’re enrolled in. Carefully read the information below.
Returning Student or CDS Expiring?
There is no need to complete another CDS form if one has previously been submitted however, an updated Tuberculin Skin Test or Chest X-Ray may be required.
Required if more than 12 months since your last 2-step or 1-step TB test:

As part of our agreements with placement organizations, it is imperative that you complete all the necessary Communicable Disease Surveillance (CDS) requirements, in the timelines identified by your program.
Please read the following carefully. Failure to comply may delay the placement component of your program as well as your program end date and graduation.
Step 1: Obtain your immunization record
- Contact your local Public Health Department to request your Immunization Records.
Step 2: Download and print the CDS form
- Download and print the CDS form which includes a Physician letter
- The process to submit your Communicable Disease Surveillance information will be different depending on what program you’re enrolled in. Carefully download your specific program CDS Form.
Step 3: Complete the CDS form jointly with a Health Care Provider (Physician or Nurse Practitioner)
- Complete “Section 1: To Be Completed by Student in Pen”.
- Bring your Immunization Record to your first medical appointment.
- A Physician or Nurse Practitioner will have to complete all remaining portions of the CDS form. Your Family Physician or local clinic can help.
- Have all your forms completed and supporting documentation obtained in preparation for submitting for a Nurse Review.
Step 4: Set up your Telus CHR Connect account
- Download the Telus CHR Connect app on your mobile device (App Store or Google Play) or open the CHR Connect website in a desktop or laptop browser at chrconnect.telushealth.com.
- Sign up for a CHR Connect account.
- After creating an account, you will see an Add Clinic page. Search for “Niagara College” and tap on the + button beside our clinic’s name: “Niagara College Health, Wellness, Accessibility Services”.
- You will then be prompted to provide your information to register as a new patient with the clinic.
- You have now created your CHR Connect account and can access your appointments and messages with our clinic through the app or website.
Step 5: Create a CDS Medical Documentation Submission appointment through Telus CHR Connect.
You will start the process of submitting your CDS form and medical documentation to us by booking a Virtual appointment with the CDS Coordinator.
Here are the steps to book the appointment:
- On your mobile device, laptop, or desktop computer, log into the TELUS CHR connect app using the account you set up in step 4.
- Tap on the Appointment menu.
- On the Appointments page, tap on the New Appointment button, then choose the options as indicated below:
Step 1 – Visit Type: VIRTUAL
Step 2 – Location: VIRTUAL
Step 3 – Reason: CDS Medical Documentation Submission
Step 4 – Type: CDS Medical Document Submission.
Step 5 – Schedule: Select an appointment time and date.
Step 6 – Review: Check the details of your appointment. Click the box beside the agreement statement. Click Confirm.
Please Note:
- You do NOT attend this appointment. This appointment only serves as a request to open the portal for documentation submission.
- Once you receive a message from the CDS Coordinator, please reply and attach a scanned PDF copy of your CDS documents. Please submit only ONE PDF file.
- Before sending your reply, ensure it includes:
- A subject line
- A brief message
- The correctly attached PDF document
Important note about scanning:
Documents scanned using a mobile device may appear unclear or be too large to attach. You are encouraged to use the Xerox scanner in the Learning Commons, or any flat-bed or sheet-fed scanner available to you.
Review time:
Please allow 48–72 hours for your documentation to be reviewed. Be sure to check your CHR Connect inbox for updates from the CDS Coordinator.
Step 6: Receive your CDS clearance
- The Nurse will review your submitted CDS forms and supporting documentation.
- If medical requirements are not met, the Nurse will contact you. Ensure a working telephone number is listed on your documentation.
- If all medical requirements are met, you will receive a CDS Clearance email to your NC email address.
- Please submit this CDS clearance email to your program placement/coordinator as per their instructions.
