1. Accessibility Intake Form
This is our standard intake form that must be completed when you come in for before your first appointment with an Accessibility Consultant.

2. Privacy and Student Behaviour Agreement
This form explains how we respect your confidentiality, and outlines some exceptions to the rule. If you have any questions or concerns your Accessibility Consultant will be happy to assist you.

3. Expectations of Students and Release of Information Form
This form outlines what is expected of students who are receiving and using our services. The form may give permission for your Accessibility Consultant to contact your teacher(s) or other NC college personnel to review your needs, and to ensure that you receive proper assistance during your studies.

4. Intellectual Property Agreement Form
This form states that you acknowledge that your professors’ material obtained by audio, video etc. will be used by you personally, and will not be shared with peers, or through social media sites and other online services.

Documenting Your Disability
Please provide your documentation to the Accessibility Consultant during your initial appointment or by sending it to our office ahead of time at [email protected].
Substantiating Your Disability Sheet
Bring this document with you to your health-care provider to help them understand what you need to document your disability.
- This sheet tells you what we need for official documentation to be able to grant you academic accommodations for a disability.
- For students who are eligible for OSAP, this sheet helps us confirm your status as a student with a disability.
Functional Limitations Assessment Form
If you do not wish to disclose your disability, bring this document to your health-care provider to fill out and sign.
- Students with a disability are not required to disclose their actual disability if they prefer not to. As an alternative, the Functional Limitations Assessment form can be filled out by an appropriate health-care provider.
- This form will document how your disability affects your daily functioning and ability to complete your program requirements.
