1) Complete the required online forms.
- Please complete the required online forms. There are 4 forms in total, including the Intake Form, Privacy and Confidentiality Form, Expectations of Students and Release of Information Form, and the Intellectual Property Agreement Form.
Once you have completed your forms, please call or visit our office to schedule an appointment with an Accessibility Consultant. If you have difficulties with the forms, please connect with our administrative team.

2) Gather documentation.
Attempt to find any supporting documentation about your condition as we require it to verify your need for accommodations.
- You are not required to disclose your disability diagnosis to register for accessibility services.
- See the How to Document your Disability link for more information on what might be appropriate supporting documentation.
3) Attend your appointment with an Accessibility Consultant.
Bring any documentation that you have with you to the appointment. Your Accessibility Consultant will work with you to develop a unique Academic Accommodations plan based on:
- The functional limitations caused by your disability in the academic environment;
- Your program and its essential requirements, and;
- Any accommodations you received previously at the secondary or post-secondary level.
You must actively participate in the development of your academic accommodation plan by working with your Accessibility Consultant and other HWAS staff as required. This responsibility involves providing appropriate documentation, and offering suggestions and information.
4) A Letter of Accommodation (LOA) will be provided.
- A Letter of Accommodation (LOA) will be sent to you, and with your consent, copies will be sent to all of your instructors, your Associate Dean, your Academic Advisor and other offices that your LOA is relevant to.
- Your LOA outlines the accommodations you require. It does not disclose your disability.
- Once informed, faculty and staff must grant your recommended Academic Accommodations unless they interfere with the essential requirements of the course.
- Unless there are changes in your condition and/or Accommodations needs, your Letter of Accommodation will be automatically renewed each term that you’re taking courses. You can still come in at any point and meet with an Accessibility Consultant if you need to.
