We kindly ask that our Spa clients fill out one or more forms to ensure that we provide you the best quality of service possible. Please bring completed forms with you to your appointment. If you cannot bring completed forms with you please arrive at least 15 minutes early allow for their completion (we have copies on hand). The below forms are fillable.
To fill out a form digitally, download it to your computer, and then open it within a PDF reader like Adobe Acrobat.
Forms only need to be completed if you are a first-time client or are receiving a treatment for the first time.
New Clients
We ask that all new clients complete the Client Health History form to ensure that we provide the best quality of service possible. Please print and complete both sides of this form. Bring completed copy of this form to your appointment.
Manicures
If you are having a hand/manicure treatment for the first time we kindly ask that you complete the Hand and Nail Care form. Please complete the first 3 fillable blocks on the first page only.
Pedicures
If you are having a foot/pedicure treatment for the first time we kindly ask that you complete the Foot and Nail Care form. Please complete the the first 3 fillable blocks on the first page only.
Hair removal
If you are having a hair removal/waxing treatment for the first time we kindly ask that you complete a Waxing Consultation form. Please complete only the top 1/3 of the first page of this double sided form.
Facial treatments
If you are having any skin care/facial service for the first time we kindly ask that complete the Skin Analysis form.
Please only complete the top 1/4 of the page. Fill in your name in the “Client Name” box, and then check Y or N next to each product listed under “Home Care” to indicate whether or not you use those products part of your home skin care routine. Under the heading “1st Appointment” please write down what product is used, if applicable and how frequently.
Massages & Back Facials
If you are having a massage for the first time we kindly ask that you complete a Massage Consultation form. Please first fill in the Client name and date then complete “Specific Problems to Treat” and check applicable conditions under “Medical History/Medications”.
