After Approval

Annual Research Project Status Report and End-of-Study Report

To report research project status and to report the completion of a project. This is an online form.

The Annual Research Study Status and End-of-Study Report Form shall be submitted as an annual status report for multi-year research studies and upon completion of all research studies. The form shall be submitted on the due date(s) indicated on the certificate of ethical clearance in order to avoid project delays/suspension.

Request for Changes to a Previously Approved Research Ethics Application

To report or request changes; unanticipated issues or events; and time extension requests. This is an online form.

The Request for Changes to a Previously Approved Research Ethics Application – Form shall be used to report any substantive changes (e.g., changes that may increase the level of risk to participants) to the last approved research protocol or significant changes to the final approved documentation by the REB; unanticipated issues or events that have/may increase the risks to participants; and research study time extension requests. If you anticipate for your research study and/or involvement with research participants to extend beyond the expiry date, you are required to submit your request at a minimum, 30 days before the expiry of your clearance certificate.
